About Wellura

 

Wellura is an innovative, premium home care service in Waterloo  Region. We're building something different - a truly integrated care model  where a Care Coordinator manages all aspects of a client's care (PSW  services, companion care, home maintenance, transportation coordination).  We're focused on preventive, coordinated care that helps seniors and adults  with disabilities maintain independence at home with dignity and safety. 

We're in our founding phase, building our initial client base before public launch,  and we need expert guidance to ensure our assessments meet the highest  professional standards.

Position Overview

We're seeking an experienced Occupational Therapist to provide expert  consultation and support as we develop and refine our Home Safety & Care  Assessment process. This is a flexible, contract-based role ideal for an OT looking  to supplement their practice while contributing to an innovative care model.

You'll work closely with Wellura's founder to conduct comprehensive in-home  assessments, provide clinical expertise on safety evaluations, and help establish  best practices for our assessment program. This role offers variety, autonomy,  and the satisfaction of directly shaping how families receive care in our  community.

Key Responsibilities

 

Client Assessments (Core Deliverable) 

  • Conduct comprehensive home safety and care needs assessments for  prospective Wellura clients 

  • Perform room-by-room safety evaluations identifying fall risks and  environmental hazards 

  • Assess functional abilities (ADLs, IADLs, mobility, cognition, safety  awareness) 

  • Evaluate need for assistive devices and home modifications 

  • Provide professional recommendations on care needs and service  intensity 

  • Document findings in clear, client-friendly assessment reports 

Clinical Consultation & Expertise 

  • Provide professional guidance on complex assessment cases (e.g.,  advanced cognitive decline, complex mobility needs) 

  • Review and provide feedback on assessment processes and  documentation 

  • Ensure assessments align with best practices in occupational therapy and  senior care 

  • Advise on assistive device recommendations and home modification  options 

  • Support care plan development with clinical expertise when needed 

Knowledge Transfer & Best Practices 

  • Share assessment methodologies and clinical reasoning approaches • Provide guidance on identifying subtle safety concerns or red flags • Recommend resources, tools, or training materials to strengthen  assessment quality 

  • Answer clinical questions as they arise 

  • Help establish standardized assessment protocols

 

Collaboration & Communication 

  • Work collaboratively with Wellura's Care Coordinator and founder

  • Communicate assessment findings clearly to clients and families 

  • Maintain professional, compassionate approach with vulnerable  populations 

  • Support smooth transition from assessment to care plan implementation

Ideal Candidate Profile 

Required Qualifications: 

  • Current registration with College of Occupational Therapists of Ontario  (COTO) 

  • Minimum 3-5 years of clinical experience in geriatrics, home care, or  rehabilitation 

  • Strong assessment skills in home safety evaluation, fall risk, and functional  capacity 

  • Excellent communication skills - ability to explain clinical concepts to non clinicians 

  • Valid driver's license and reliable transportation for home visits 

  • Professional liability insurance 

Preferred Experience: 

  • Previous work in home and community care settings 

  • Experience with seniors, dementia/cognitive decline, and mobility  impairments 

  • Knowledge of assistive devices, home modifications, and community  resources 

  • Familiarity with Home and Community Care Support Services (CCAC/LHIN  system) 

  • Experience training or mentoring others in assessment practices 

  • Understanding of person-centered care approaches

Work Arrangements 

Schedule & Flexibility: 

  • Flexible scheduling - you control your availability 

  • Part-time commitment - estimated 10-15 hours per month initially

  • Home visits scheduled at mutually convenient times (typically weekday  mornings or early afternoons) 

  • Potential for growth - hours may increase as Wellura scales

  • No on-call requirements - purely scheduled assessments 

Typical Work Pattern: 

  • Conduct 3-5 comprehensive home assessments per month (90-120 min  each) 

  • Assessment report documentation (30-45 min per assessment) 

  • Occasional consultation calls or meetings (1-2 hours per month)

  • Ad-hoc clinical consultation as needed 

Geographic Scope: 

  • Primarily Kitchener, Waterloo, Cambridge, Guelph 

  • Occasional visits to surrounding townships 

  • Mileage reimbursement for client visits 

Financial

  • Competitive hourly rate commensurate with experience 

  • Mileage reimbursement at CRA rates for client home visits 

  • Flexibility to negotiate project-based fees if preferred 

  • Invoicing monthly via simple proces

 

Contract Terms: 

  • Independent contractor arrangement 

  • No benefits or statutory deductions (you’ll manage your own  taxes/business expenses) 

  • Flexible commitment - either party can adjust hours or end arrangement  with 30 days' notice 

  • Professional services agreement outlining scope, confidentiality, and  liability 

  • Professional liability insurance required (you maintain your own coverage)

  3. Contract & onboarding - formalize agreement, review assessment  protocols 

  4. First assessments - shadow on 1-2 assessments together, then  independent work

Q: How many assessments would I typically do per month? 

A: Initially, 3-5 per month. This may increase to 8-10 as we grow, but we'll scale  based on your availability. 

Q: What if I'm already working full-time elsewhere? 

A: That's fine, as long as you have some flexibility for morning and early  afternoon time M-F. This role is designed to fit around other commitments. You  control your schedule and accept assessments when available. 

Q: Do I need to provide my own assessment tools? 

A: Wellura provides a basic assessment toolkit (clipboard, forms, measuring  tape). You're welcome to bring additional tools you prefer to use.

 

Q: What if a case is too complex for my scope? 

A: You provide recommendations based on your professional judgment. For  cases requiring specialist intervention (e.g., advanced dementia), you'd  recommend referral to appropriate specialists. 

Q: Is this role supervision or teaching? 

A: No, this is clinical consulting. You're the expert conducting assessments and  providing professional recommendations. You'll work collaboratively with our  team, sharing your expertise naturally through the work. 

Q: Can I do this through my existing OT professional corporation?

A: Yes, if you have a professional corporation for your OT practice, we can  contract with your corporation instead of you personally. 

Q: What documentation do I need to provide? 

A: You'll complete a comprehensive assessment report (template provided) that  includes safety findings, functional assessment, and care recommendations. The  reports are client-facing and should be clear, actionable, and professional. 

Q: What happens if a client doesn't sign up with Wellura? 

A: You still get paid for the assessment. Your role is to provide professional  evaluation and recommendations - the business decision is up to the client.
